At Osteo 3730 we value your privacy, as a result your health information is confidential and handled in accordance to the Clinic Privacy Policy. This Policy aligns with the Privacy Act (1998) and provides information that explains how we collect, use, store and protect your information. It is also to inform you of your rights in relation to the information that we collect.

Collection & Use

It is a government requirement that Osteopaths keep clinical records. This information is used for the purpose of diagnosis, treatment and management of your condition.

In order to provide the best possible patient care you may be asked to provide information regarding things such as relevant health history, family history and contact information such as name, address, date of birth, gender. This information may be collected by your Osteopath verbally or obtained via the New Patient Intake form or online booking system.

We may at times collect additional information including Medicare or Private health details for the purpose of billing and/or rebates. Depending on the purpose of your visit, this may also include details relating to the workplace or relevant insurance company. We also collect your information via communication avenues such as website, email, fax, post, text, social media and telephone for the purpose of clinic management and patient care.

Although most of this information is obtained directly from you, we may at times collect information from other sources such as another health professional, hospital or diagnostic imaging service (as required.) Your information will be used to enhance patient care and as part of standard business procedure, and will not be used for any other purpose without your consent.

Storage

Clinical records are stored and transferred securely to safeguard against theft, use, modification and disclosure. Some of the systems we have in place to protect your information include passwords and encryptions, secure premises, tech protection software and safe disposal. This means at all times information stored by Osteo 3730, whether in hard copy or electronic form, is handled with the upmost care.

You have the right to access your information that is held with us. Should you require access to this information you will be asked to submit your request in writing. You may also access a hard copy of our Privacy Policy whilst at the clinic.

Should it be identified that there is any inaccuracies in the information that is held about you, will we take reasonable steps to correct this information provided it does not breach our legal or ethical requirements.

If at any time you have concerns or complaints in regards to our Privacy Policy or the information we hold about you, we will ask that you provide this to us in writing. Within reason, we will endeavour to resolve any concerns or complaints that may arise.
